Atosiban (trade name Tractocile®) is a synthetic peptide drug, oxytocin analog, that can competitively bind to oxytocin and vasopressin V1A receptors, thereby blocking the pathway of action of oxytocin and vasopressin , reduces uterine contractions, and is mainly used clinically to delay imminent preterm labor.

Atosiban is a disulfide-bonded cyclic polypeptide composed of 9 amino acids, which is similar in structure to oxytocin, but modified at positions 1, 2, 4, and 8 on the molecular structure of oxytocin. Among them, the N-terminal deamination of cysteine residue at position 1 generates 3-mercaptopropionic acid, L-tyrosine at position 2 is modified to D-tyrosine, ethoxy is substituted for phenol, and threonine at position 4 is substituted for glutamine. Amide, ornithine at position 8 replaces leucine.

Atosiban, an oxytocin receptor antagonist, binds to the oxytocin receptor bound to the muscularis membrane, preventing the oxytocin-stimulated increase in inositol triphosphate production. This ultimately prevents the release of stored calcium from the sarcoplasmic reticulum and the subsequent opening of voltage-gated calcium channels. This shutdown of intracellular calcium increases prevents uterine muscle contractions, reduces the frequency of contractions and induces uterine quiescence, thereby allowing smooth muscle relaxation for the treatment of preterm labor.
